Legal Name Fields

A student’s legal name in its four parts, the order an ID and DPS write it: First name with Middle initial beside it, then Last name with Suffix. The two short parts take two fifths of the row. Every form that takes a student’s name draws this block: checkout, the road-test booking, Add student and Edit details.
